Pregnancy Care Management

Pregnancy Care Management services are available to pregnant and postpartum women receiving Medicaid who have risk factors which may put them at risk for poor birth outcomes.  Pregnancy care managers work in partnership with Pregnancy Medical Homes, OB office providers, and community resources to ensure proper care during the pregnancy.

 

The goal of the Pregnancy Care Management Program is to improve birth outcomes through coordinated, evidence-based maternity care for women at risk.  Care Managers are Registered Nurses or Social Workers who develop a plan of care with goals and interventions for each patient, to encourage a positive pregnancy and childbirth experience, and address newborn care and parenting concerns.
