Child Health Services
Involves all aspects of pediatric assessment and health care for patients to 18 years of age and for those patients with Medicaid to 21 years of age.
Senior Services
This division provides in-home care to elderly and disabled individuals wishing to remain at home, because, truly, "there's no place like home..."
Family Care Coordination Services
This program provides case management for high-risk pregnant women and children up to 5 years of age and postpartum/newborn home visiting by an R.N.
These services include the Supplemental Nutrition Program for Women, Infants, and Children (WIC), Breastfeeding Promotion and Support, Community Nutrition Education, Medical Nutrition Therapy, and Diabetes Self Management Education. Our nutrition services are designed to promote healthy lifestyles for people of all ages.
Community Health Services
These services include Immunizations, Adult Primary Care, Migrant Farmworker Program, Family Planning, STD/Communicable Disease, and other services for the protection of the community's overall health.
Rabies Control Section
This section helps prevent the spread of disease by assuring that the public is protected from environmental disease spread by unvaccinated, dangerous, and/or wild animals.
Health Promotion/Health Education
This section provides health promotion and disease prevention services at schools, worksites, and the community.
Environmental Health Division
The principal function of this section is to prevent disease by ensuring that no unsafe food or water is consumed.
